Six month post afib ablation. I'm having more ectopics lately. One a minute at times, or runs of five or more separated by 3-5 normal beats. Nothing in my life has changed so it's a pretty random thing. I get them for a window of a few hours, typically early afternoon from 1:00 - 5:00 pm, but they can start around 10:00 am as well. I waited three months before starting light exercise. have had bouts of ectopics and they do seem worse in the afternoon if I push my workouts too hard. I have heard that most (if not all) follow-up ablations are RF. Cryo can't get to some of the smaller problem areas and can only hit the main pulmonary vein. Therefore any follow-ups are done via RF. Someone can correct me if I'm wrong here. Generally speaking, cryo has no benefits over RF other than it's easier for the physician to perform. I had a PVC ablation (September 2018) a year before my afib ablation (December 2019). I don't know the technique he used although I'm fairly certain it was stereotactic. PVCs were causing me much more trouble initially and afib was rare. I would get terrible ventricular ectopics during work outs. So we did the PVC ablation first. I had my afib ablation almost three months ago. No breakthrough episodes but I do get PACs from time to time. My EP has had me on Flecainide the entire time (100 mg two times a day), and we started Metropolol 12.5 mg once a day about a month ago. My HR is slower after the ablation. Not a lot, but maybe 10%. I'm around 55-60 BPM. But can get to 50 pretty commonly. I was closer to 65 pre ablation. granted I am on Flecainide and Metropolol still, but my rate never went up after the procedure, even before I started taking meds again a day or so later. 35 is really low though. I'm with Pompon. Through experience I've learned I'm more "primed" for afib if I've not slept well or am stressed. My triggers at that point are very random. Meaning a cold drink could set me into afib, or not. Rising from a seated position or exerting myself too much from rest could set things off, or not. I'm ten weeks post ablation for afib. I'm still on 100 mg Flecainide twice a day and 12.5 mg of Metropolol. I suffered from afib about 1-2 times a year for ten years and then in the last year was in it several times a month and then the last month, was going into it every day for a few hours. Agree with Carey. Check with you EP, but mine kept me on Flecainide for month post procedure for starters and is keeping me on it for another couple months as a precaution. This will cover the entire blanking period. Went to see my EP at the five week post ablation mark. I mentioned all the afternoon ectopics and he said he sees a higher than pre ablation burden of PACs in about 20% of his patients. Not a high percentage. The afternoon only thing for me is a little unique. Mind you this is the top EP at the top cardiac facility in St. Louis. He does perhaps thousands of these a year. I too am vagal. I get ectopics in the middle of the night and at times, and have sometimes been able to tie it to my not breathing well. I'm often lying on my back and I catch myself mouth breathing almost as a last resort, like I "puff" through my mouth. Call it sleep apnea I guess. I'm just about at four weeks post ablation. Can't really say my afternoon ectopics are settling down. I feel great most of the day and can workout comfortably in the morning, but I can get long (twelve or more) runs of single ectopics beat separated by single pauses if I just get up to walk around in the afternoon. Cortisol is an interesting point by you both. I find a tiny bit of stress (mental or even a very tiny bit of activity) can trigger ectopics, but if I walk briskly or drift into more of a cardio situation, I rarely have ectopics. Was reading about it a little and cortisol is highest in the morning and high when under stress. Interesting, my ectopics are very low in the morning and high late in the day when under stress. I'm pretty new to all this myself, but it sounds like you have adrenergic afib, meaning it fires up when adrenaline kicks in. Although you were resting in a couple cases, and going into it at rest is more indicative of vagal afib, it was being jolted to an awakened state that set you off. But the GERD is interesting.
