Try the 90mcg regular dose of Vitamin K2-MK7 and monitor your INR carefully each week for the first few months either by weekly INR tests at the lab or, better yet, with a well calibrated Home INR monitor that you know how to use properly.
I suspect you will find that even at 90mcg per day of K2-MK7 your INR will not move up or down more than about +/- 0.2 INR compared to taking 45mcg of K2-MK7 or taking no supplemental Vitamin K2 -MK7 at all. You should easily be able to titrate your dose of Coumadin to stay well within your Sweet spot between 2 and 3 or 2.5 and 3.5 which ever your Doc has you using for your optimal INR range.
The 90mcg will offer more systemic benefits overall too and should help keep your iNR levels more stable than when taking no Vitamin K2-MK7 at all.
